WYFC CODE OF CONDUCT
Players and parents must follow the Code of Conducts which can be found here.
MEDICAL CONSENT
In the event of an accident/injury where the coach is unable to contact the named on the left, I give permission for the coach to sign the authorisation for any medical treatment/ procedure which may be required. I also consent to qualified first aiders offering treatment if required. Parents are responsible for their children at all times. If you plan to leave the ground, then you must arrange for another adult to take responsibility & inform the coach. This is vital in event of sickness or accident.
MEMBERSHIP FEES
The non-refundable membership fee is £30 and will be taken with the initial payment.
All payments are to be made through LoveAdmin. Existing players will use their current accounts and new players will be asked to register through LoveAdmin to set-up payment.
All Players are required to send an up to date picture for their player’s card. This must be sent to the team manager.
**NEW PLAYERS**
If you are new to Ware Youth FC, you must also provide the players proof of age i.e. passport or birth certificate.
THE PLAYER WILL NOT BE REGISTERED WITHOUT THESE CONDITIONS BEING MET.
*Herts FA require this information & your child cannot be registered without it.
By submitting this registration, I (the parent/guardian) consent to the Ware Youth FC code of conduct (found on our website and available on request) and medical consent.
